Report Fraud, Waste, and Abuse

The form below will enable you to report concerns about fraud, waste or mismanagement. Simply enter your information and comments, then press the "Submit" button to deliver the message. If you decide to report anonymously, please provide sufficient details, i.e., names, dates, organizations, contract numbers etc., to permit our office to investigate your complaint or issue.

***The DIA OIG can only accept UNCLASSIFIED complaints on the NIPRnet system. Please do not provide any classified information higher than the category (UNCLASSIFIED or FOUO) in this complaint form. Classified complaints must be made via classified networks, a secure communications device, or in person.

The DIA OIG Hotline is a confidential channel for reporting information if you suspect fraud, waste, or abuse related to DIA programs, personnel, and operations. You can report to OIG Hotline if your allegation(s) potentially involve the following: Bribery and acceptance of gratuities, Conflicts of interest, Unauthorized use of DIA technology systems, Government travel or purchase card fraud, Cost or labor mischarging, Health and safety issues, Contract and procurement fraud, Leaks of classified information, Abuse of authority or power, Time & Labor fraud, and Violations of policy, procedures, and regulations.

DATA REQUIRED BY THE PRIVACY ACT OF 1974

PURPOSE: To obtain sufficient information to inquire into matters presented and to provide
appropriate responses, referrals, or inquiries, where deemed appropriate.

ROUTINE USES: Information is used for official purposes only within the DIA Office of Inspector
General, to answer complaints or respond to requests for assistance, advice, or information; by
members of Congress and other government agencies when determined by the DIA OIG to be in the
best of interest of the Intelligence Community.

Definitions of confidentiality:

I choose to waive confidentiality and I give consent for DIA OIG to release my identity outside the OIG on a need-to-know basis. In waiving my confidentiality, I understand that I may be able to assist DIA OIG in addressing my allegations of wrongdoing. I also understand that no action constituting a reprisal, or threat of reprisal for making a complaint or disclosing information to the OIG may be taken by any employee in a position to take such action, unless the complaint or information was disclosed with the knowledge that it was false.

I request confidentiality. I choose to identify myself to DIA OIG for the purpose of making this submission, but I DO NOT given consent to OIG to provide my name and contact information outside the DIA OIG except as required by law or unless the DIA IG determines that such disclosure is unavoidable during the course of an investigation.

I choose to provide my complaint ANONYMOUSLY. I understand that this may limit DIA OIG’s ability to conduct an inquiry, if one is warranted, or to appropriately address my allegations or obtain additional details.
